Table type questionnaire

Dear All,
Is there any way to create a xlsx table form using odk2.0. I have attached a sample question.

You can definitely program in something equivalent with ODK 2. The XLSX converter reference is here: ODK-X XLSX Converter Reference — ODK-X Docs and describes how to set up the files in excel including different question types. If, as it looks, you want to have repeated entries, which are subforms or linked tables: Using ODK-X XLSX Converter — ODK-X Docs
